How to track semaglutide (Ozempic, Wegovy, Rybelsus)

To track semaglutide, log every dose with the date, time, product and form exactly as on the box (Ozempic, Wegovy or Rybelsus), the dose your prescriber set and, for injections, the site. Write down the date you first use each pen, because the label limits how long an opened pen can be kept, and record your weight and any side effects on a fixed day each week. The US labels describe the injections as once weekly and the tablets as once daily.

What to log for each dose

One row per dose, including a skipped one. The fields fit the weekly injections; tablets are covered underneath.

Missed or late doseThe date the dose was due and the date you took it, or "skipped"The labels' missed-dose window depends on the days that have passed.

Tablets (Rybelsus, Wegovy or Ozempic tablets) are labelled once daily, on an empty stomach in the morning with up to 4 ounces of water, waiting at least 30 minutes before eating, drinking or other oral medicines. Log the time you took the tablet and the time you first ate or drank.

Ozempic, Wegovy or Rybelsus: what changes in the log?

Same ingredient, different log. For weekly Ozempic or Wegovy injections the key fields are the dose day, the site and the pen's first-use date. For daily tablets they are the time taken and the first food or drink. Write the brand and form exactly as on the carton, so a switch between products shows in your history. For a compounded vial, also record the pharmacy, the label concentration and the lot.

Checking a compounded vial: mg to units

Ozempic and Wegovy pens and syringes deliver a set dose, so there is nothing to convert. A compounded vial is different: its label gives a concentration in mg per mL, and FDA has reported patients taking several times their intended dose after confusing mg, mL and units. Enter the concentration on your label and the amount your prescriber gave you, and check the result with your pharmacist.

Storage and in-use dates (from the label)

Ozempic pen: refrigerate at 2°C to 8°C (36°F to 46°F) before first use. After first use it can be kept for 56 days at 15°C to 30°C (59°F to 86°F) or in the refrigerator. Do not freeze; protect from heat and sunlight.

Ozempic prefilled syringes and Wegovy single-dose pens or syringes: refrigerate; if needed, up to 28 days at 8°C to 30°C (46°F to 86°F), in the carton to protect from light. Wegovy FlexTouch: 56 days after first use.

Tablets: 20°C to 25°C (68°F to 77°F) in the original bottle, away from moisture. Log the first-use or out-of-fridge date and the discard date that follows (see tracking expiry dates).

Half-life (from the label)

Section 12.3 of the Ozempic label gives an elimination half-life of approximately 1 week, and says semaglutide is present in the circulation for about 5 weeks after the last dose. The Wegovy label gives the same half-life and says about 5 to 7 weeks after the last dose. The Rybelsus label also says approximately one week.

It describes how the drug leaves the body, not when to take a dose. Enter 1 week in the half-life calculator to see the curve.

What to record about side effects

The Ozempic label lists nausea, vomiting, diarrhea, abdominal pain and constipation as the most common adverse reactions. Record each with the day it started relative to dose day, a 1 to 5 severity and its duration, next to the current dose step. What to share with your doctor turns this into an appointment summary.

Red flags: when to contact a clinician

From the semaglutide Medication Guides; follow the one that came with your medicine.

  • Stop and call your healthcare provider right away for severe pain in your stomach area (abdomen) that will not go away.
  • Get medical help right away for symptoms of a serious allergic reaction, including swelling of your face, lips, tongue or throat, or problems breathing or swallowing.
  • Tell your provider about a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath.
  • Tell your provider right away about upper stomach pain, yellowing of your skin or eyes, fever or clay-colored stools (possible gallbladder problems), or nausea, vomiting or diarrhea that does not go away.
  • Tell your provider about changes in vision, and learn the signs of low blood sugar if you also take insulin or a sulfonylurea.

You can report a side effect to FDA through MedWatch or at 1-800-FDA-1088.

Semaglutide tracking questions

Is there an Ozempic tracker app?

PeptideMate works as an Ozempic tracker: you type the medicine name as it appears on your pen, set a weekly reminder for your dose day and log each dose with its injection site. It is free for Android, iPhone, iPad and Mac and in the browser. PeptideMate is an independent app and is not affiliated with Novo Nordisk.

How do I know when my Ozempic pen has to be thrown away?

Record the date you first use each pen. The Ozempic label says an opened pen can be kept for 56 days, so the discard date is 56 days after first use, even if there is medicine left. Add it as a note or reminder.

What should I do if I miss my weekly semaglutide dose?

Follow the missed-dose instructions in the Medication Guide for your product, or ask your pharmacist. They depend on how many days have passed. Logging the date the dose was due and the date you took it makes the answer easy to work out.
